Log into the Green Dot app. Go to Settings, Manage Card. Select Report Card Lost/Stolen. Enter requested info.

Bring the barcode and your government-issued Photo ID to Walmart to pick up your cash. A $3 service fee applies.
Limits: $20 - $500

Once you report your card lost or stolen, your card will be deactivated and a new one sent. A $5 card replacement fee will apply. See your Account Agreement.
